Thunder Bay, Ontario (January 29, 2025) — NorWest Community Health Centres (NorWest CHC), is excited to announce its upcoming schedule offering a range of Self-Management Programs including workshops and training programs aimed at supporting individuals living with chronic conditions. Start the new year off right by taking control of your health with the Chronic Disease Self-Management Program (CDSMP) workshops or the Cancer “Thriving and Surviving” Self-Management Program Workshops.

“2024 was a year of growing capacity across NorthWestern Ontario to increase knowledge and access to Chronic Disease Self-Management Programs,” said Kevin Ng, Manager for CDSMP. “In 2025, we’re thrilled to offer a full lineup of workshops and training designed to empower individuals to actively engage in their health and well-being.”

Chronic Disease Self-Management Program (CDSMP) Workshop – These workshops are tailored to empower individuals living with chronic conditions, providing practical strategies and support to enhance quality of life and reach personal health goals. NorWest CHC will host CDSMP Workshops both virtually and in-person in Fort Frances this month. This six-session, six-week program equips participants with the knowledge, skills, and confidence to better manage their ongoing health issues. The workshops are open to anyone living with chronic diseases such as diabetes, arthritis, heart disease, and lung disease.

Cancer: Thriving and Surviving Self-Management Program Workshop – Next month, NorWest CHC will launch a virtual series for the Cancer: Thriving and Surviving Self-Management Program. This workshop series is designed for those living with or affected by cancer, helping participants manage their health, stay engaged in their lives and relationships, and develop a more positive outlook for the future. Like the CDSMP workshops, this series will consist of six sessions.

Diabetes Self-Management Program Training will take place from March 24 to March 27, 2025, at NorWest CHC in Thunder Bay. This training is open to diabetes clients and caregivers, healthcare providers, community leaders, and anyone interested in facilitating diabetes self-management workshops in their communities.

“Chronic diseases are a significant concern in Ontario, accounting for about 70% of deaths and deeply affecting individuals and families,” Ng adds. “Implementing effective self-management strategies can help those with chronic conditions lead healthier lives and improve their overall quality of life.”
